Both. We run new implementations and pick up existing ones that need upgrades, fixes, extensions, or support. For an existing system we start with a short review of its configuration, customisations, and integrations before proposing any changes.
Migrations are reconciled and audited against the source system, rehearsed before cutover, and come with a rollback plan. Finance runs a parallel close so the numbers are checked before the old system is switched off.
That's the goal of how we build. We configure before we code, and when code is needed we write it as extensions rather than changes to the core product, so a version upgrade doesn't overwrite it.
